Abstract

Background. Meningiomas of the brain, which develop from the meninges, account for approximately 30% of all CNS neoplasms. They are predominantly seen in women, especially those aged 30 to 60 years. Meningiomas are classified into three main grades based on histotype: G1 – benign, slow-growing, G2 – atypical, and G3 – anaplastic. The primary treatments for meningiomas are surgical removal and radiation therapy. Stereotactic radiosurgery/radiotherapy (SRS/SRT), a new, high-tech radiation technique, is an important method for managing tumor growth, particularly in cases where surgery could lead to serious consequences for the patient. The effectiveness of radiation therapy may vary depending on the type of meningioma, highlighting the need to study these differences. Purpose of the study. To determine the comparative efficacy of stereotactic radiotherapy for G1 meningiomas by analyzing clinical cases of fibrous and meningotheliomatous meningiomas. Materials and Methods. The study included 12 patients with G1 meningiomas: 6 with fibrous meningiomas and 6 with meningotheliomatous meningiomas. In the fibrous meningiomas group, 3 patients received SRS with a single focal dose (SFD) of 16 Gy, and 3 others received SRT with a SFD of 5.5 Gy (SFD of 27.5 Gy). In the meningotheliomatous meningiomas group, 2 patients received SRS with a SFD of 16 Gy, and 4 patients received SRT (SFD of 5.5 Gy, SFD of 27.5 Gy). Outcomes were assessed at 3, 6, 9, and 12 months using contrast-enhanced MRI. Results and Discussion All six patients with the meningotheliomatous form achieved complete regression, while among patients with the fibrous form, partial regression was observed in one patient and stabilization in five. Conclusion: Meningotheliomatous meningiomas demonstrated greater sensitivity to stereotactic radiotherapy compared to fibrous forms, with a higher rate of complete regression.
