Математическое моделирование стратегий вакцинации COVID-19 в Кыргызстане и нагрузка на систему здравоохранения

Аннотация

В декабре 2020 года во всем мире началась беспрецедентная вакцинация против COVID-19. Поскольку в настоящее время в большинстве стран запасов вакцины недостаточно для вакцинации всего населения, нуждающегося в вакцинации, определение эффективных стратегий вакцинации может помочь снизить нагрузку на систему здравоохранения. Имеющиеся данные свидетельствуют о том, что тяжесть заболевания и смертность различаются в зависимости от возраста, поэтому вакцинацию, зависящую от возраста, можно рассматривать как важную и приоритетную стратегию. С помощью детерминированной модели SIERS, включающей данные по возрасту, в статье сравниваются четыре гипотетические стратегии вакцинации, зависящие от возраста, и их влияние на эпидемию в Кыргызстане. В краткосрочной перспективе (до марта 2022 года) основное внимание будет уделяться возрастным группам высокого риска (старше 50 лет) с некоторым распространением вакцинации на группы с высокой заболеваемостью (20-49 лет), что может привести к снижению случаев с выраженной симптоматикой и смертности, обусловленной COVID-19. С другой стороны, влияние на общее оценочное число случаев COVID-19 при сравнительно небольшом охвате групп с высокой заболеваемостью (15-25% только на основе текущей доступности вакцины) не будет столь существенным. Приведенные выше результаты и новые данные о вакцинах свидетельствуют о том, что вакцинация более эффективна в борьбе с эпидемией и снижении смертности, если она поддерживается немедикаментозными вмешательствами. В краткосрочной перспективе сосредоточение внимания на группах высокого риска может снизить нагрузку на систему здравоохранения. Однако эффект сдерживания очередного пика может быть достигнут только за счет более широкого охвата групп с высокой заболеваемостью.
