Информативность различных образцов патологического материала при лабораторной диагностике туберкулеза у детей

Аннотация

Туберкулез (ТБ) у детей характеризуется частотой внелегочной локализации процесса, редкостью и скудностью бактерио выделения. Всемирная организация здравоохранения (ВОЗ) рекомендует с целью повышения эффективности диагностики ТБ проводить исследования любого доступного диагностического материала всеми доступными лабораторными методами. Целью настоящего исследования явилось изучение информативности различных видов образцов диагностического материала для детекции M. tuberculosis у детей. Материал и методы. Ретроспективный анализ результатов микробиологических исследований на микобактерии туберкулеза 1676 детей в возрасте до 15 лет за период 2017 - 2021 годы, выполненные в Республиканской референс лаборатория Национального центра фтизиатрии. Результаты. Наиболее информативными образцами явились био- птаты/резектаты, содержимое свищей, промывные воды бронхов и ликвор. Чувствительность быстрых молекулярных методов диагностики ТБ (Xpert MTB/RIF, GenoTypeMDRTB plus) была выше по сравнению с результатами культуральных (MGIT, ЛЙ) и микроскопии. Заключение. Совокупная информативность различных видов образцов диагностического материала при лабораторной диагностике ТБ у детей составила 11,4%. В качестве начального диагностического теста рекомендуются быстрые молекулярные методы для верификации диагноза ТБ и старта адекватного его лечения.
