Является ли благоприятным назначение метформина у пациентов с хронической сердечной недостаточностью ишемической этиологии на этапе инсулинорезистентности?

Аннотация

Хроническая сердечная недостаточность (ХСН) и сахарный диабет (СД) 2 типа часто сосуществуют. ХСН в настоящее время рассматривается как состояние инсулинорезистентности (ИР) и ассоциируется с ухудшением прогноза и качества жизни пациентов. Благоприятное влияние метформина на течение сердечно-сосудистых заболеваний, ХСН в частности, у пациентов с СД 2 типа, установлено в ходе многочисленных исследований. Целью настоящего исследования является оценка влияния метформина на клиническое течение ХСН ишемической этиологии у пациентов на этапе ИР. Материал и методы. В ходе открытого, проспективного рандомизированого клинического исследования 76 пациентов с ХСН ишемической этиологии (средний возраст 62 (57,5-65) лет, мужчин (n=44), средняя фракция выброса левого желудочка (ФВ ЛЖ) 44,0% (34-52,5)), были рандомизированы на 2 группы: с приемом метформина (n=39) и без приема (n=37), соответственно. Всем пациентам выданы и подробно разъяснены принципы модификации образа жизни. ИР диагностирована при HOMA-IR ≥ 2.5. Не- обходимые объективные (с опросом о качестве жизни пациентов с ХСН), лабораторные и инструментальные методы исследования, 6-минутный тест с ходьбой были провены исходно и через 12 месяцев наблюдения. Результаты. Терапия метформином ассоциировалась с улучшением таких ХСН-ассоциированных клинических индикаторов, как: ИР (HOMA-IR), показатели альдостерона, функциональная способность и качество жизни. Также в группе с метформином отмечалось достоверное улучшение почечной функции (по показателям креатинина и суточной протеинурии) и средней ФВ ЛЖ. Заключение. Возможности благоприятного влияния метформина на течение ХСН ишемической этиологии у пациентов без сопутствующего СД 2 типа позволит оптимизировать стратегию вмешательств для улучшения качества жизни и долгосрочного прогноза при ХСН, а также снизить частоту конверсии в СД 2 типа.
