Особенности жирового обмена при неалкогольной жировой болезни печени у больных с различной массой тела в высокогорных условиях Кыргызстана

Аннотация

Цель. Изучить особенности нарушения липидного обмена при неалкогольной жировой болезни печени у больных с различной массой тела в условиях обжитого высокогорья. Материал и методы. Исследование про- водилось в рамках проекта «Этиопатогенетические особенности и темпы развития неалкогольной жировой бо- лезни печени (НАЖБП) в условиях Кыргызстана» (№ госрегистрации МЗН/ТЗ-2020-3). Проведено открытое сравнительное исследование больных с двумя формами НАЖБП: жировой гепатоз (ЖГ) и неалкогольный стеа- тогепатит (НАСГ) (n=236), проживающих в условиях низкогорья (г. Бишкек, высота над ур.м. – 750-800 м., n=111) и обжитого высокогорья (Ат-Башинский район, Нарынская область, высота над ур.м. – 2046-2300 м., n=125) Кыргызстана. Учитывая, что генетические факторы могут играть роль в развитии НАЖБП, анализировалась по- пуляция, представленная только этническими кыргызами. Средний возраст больных составил 55,7±0,95 лет. В каждой группе больных рассматривались худая (ИМТ≤23) и тучная (ИМТ >23) группы. Для определения физической активности был применен опросник физической активности (ФА), который был составлен на основе ма- териалов International Physical Activity Prevalence Study www.ipaq.ki.se. Физикальное обследование включало в себя измерение антропометрических параметров (рост, масса тела, окружность талии (ОТ)), расчет индекса массы тела (ИМТ), тощей массы тела (ТМТ), процента жира в организме. Согласно градациям ВОЗ оценивали степени ожирения по ИМТ для азиатов. Забор образцов крови для исследований производился утром натощак после не менее чем через 12 часов голодания. Производилось определение следующих показателей: глюкозы, липидного спектра (общего холестерина, ХС ЛПВП, ХС ЛПНП, триглицеридов), уровней АЛТ, АСТ. В качестве предиктора для оценки развития фиброза печени у больных НАЖБП была использована шкала BARD [1]. Система подсчёта баллов включала три переменные: ИМТ, АСТ/АЛТ и наличие СД 2. Диагноз НАЖБП ставился на основании анамнеза, лабораторных исследований, УЗИ печени и исключения других заболеваний печени. Анализ полученных результатов проводили с помощью статистического пакета прикладных программ SPSS 16.0 для Windows. Значение «p»<0,05 считалось статистически значимым при уровне достоверности 95%. Результаты. Было вы- явлено, что жители высокогорья с НАЖБП представлены более низким ИМТ относительно низко горцев. У женщин значительно и статистически значимо коррелируют уровни ТМТ и процента жира (r = - 0,971, р<0,001), тогда как у мужчин эти два показателя не связаны. У мужчин отмечена тенденция к более высоким показателям про- цента жира независимо от массы тела и региона проживания. У женщин этот показатель находился в пределах приемлемых границ и не превышал 31%. Обнаружено статистически значимое отличие уровня общего холесте- рина между низкогорцами и горцами в группе полных пациентов (р<0,001), страдающих НАЖБП. Выявлены статистически достоверные низкие показатели АЛТ в группе полных пациентов, проживающих в условиях обжитого высокогорья. Выводы. В совокупности наши результаты предполагают, что хроническая высокогорная гипоксия может замедлить течение НАЖБП, вызванной избыточным весом.
