Распространённость ультразвуковых признаков жирового гепатоза и сочетанных патологий у стационарных больных в Кыргызстане

Аннотация

Введение. УЗИ, как неинвазивный и легкодоступный инструмент, играет важную роль в диагностике неалкогольной жировой болезни печени (НАЖБП). Распространенность НАЖБП ранее не изучалась в Кыргыз- стане. Знание частоты встречаемости жировой инфильтрации печени у больных стационарного профиля необхо- димо для активной диагностики и своевременного предупреждения осложнений НАЖБП. Цель исследования- Определить распространенность жирового гепатоза и ассоциированных патологий органов брюшной полости у больных стационарного профиля. Материал и методы. Всего обследовано 6720 посетителей городской клинической больницы №1 г. Бишкек. Сред- ний возраст составил 45,9 лет. Среди них женщин – 53,8%, мужчин – 46,2%. Критериями включения в группы обследованных явились: возраст от 18 до 92 лет; согласие пациента принимать участие в исследовании. Крите- риями исключения: наличие тяжелых сопутствующих соматических, онкологических, гематологических и пси- хических заболеваний; несогласие принимать участие в исследовании. Контрольную группу составили лица без УЗ-признаков жировой инфильтрации печени (всего 2466 человек). В группу сравнения вошли 4254 человек с ультразвуковыми признаками, характерные для жирового гепатоза. В каждой исследуемой группе рассматрива- лись две возрастные категории: старше и младше 45 лет. Всем обследуемым проводилось ультразвуковое иссле- дование органов брюшной полости. Анализ полученных результатов проводили с помощью статистического пакета прикладных программ SPSS 16.0 для Windows. Результаты. Было выявлено, что среди пациентов многопрофильной больницы распространённость признаков жирового гепатоза составляет 63,3%, среди лиц младше 45 лет - 43%, а у лиц старше 45 лет - 79,1%. Среди ко- морбидной патологии у лиц с жировым гепатозом статистически значимо преобладали признаки хронического холецистита, панкреатита, желчекаменной болезни (ЖКБ) и пиелонефрита. Более 60% младшей возрастной группы с жировым гепатозом имеют признаки хронического панкреатита против 36,2% без жировой инфильтрации. Среди пациентов младше 45 лет преобладала I степень жирового гепатоза. У пациентов старше 45 лет статистически значимо преобладали II и III степени. Найдена статистически значимая корреляция средней силы жирового гепатоза с панкреатитом. Также найдена слабая прямая корреляция жирового гепатоза с хроническим холециститом и пиелонефритом. Выводы. Высокая распространенность УЗ-признаков жировой инфильтрации печени (79,1%) среди обследован- ных больных старше 45 лет, находящихся на стационарном лечении в многопрофильной больнице, и преоблада- ние у них признаков хронического холецистита, панкреатита, ЖКБ и пиелонефрита подчеркивают необходимость тщательной диагностики заболеваний печени с целью предупреждения осложнений в виде цирроза печени и гепатоцеллюлярной карциномы.
