The role of perioperative rehabilitation of patients in surgical treatment of tumors of the pelvic bones and lower extremities with endoprosthetics

Abstract

Relevance. The main method of treatment for tumor lesions of the pelvic bones and lower extremities is resection of the affected bone with endoprosthetics. Functional results after oncological endoprosthetics remain unsatisfactory in 35-40% of cases. The need to improve functional results is a significant medical and social problem. Purpose of the study. To assess the impact of perioperative rehabilitation on functional results in cancer patients who underwent surgical treatment with resection of the pelvic bones and lower extremities with endoprosthetics. Materials and methods. A comprehensive analysis of the treatment results was carried out for 348 patients who underwent resection of the affected part of the pelvic bones and lower extremities with endoprosthetics. Of these, there were 150 men (43.1%) and 198 women (56.9%), the average age was 51.3 (19-82) years. The distribution of patients by the volume and type of surgical intervention was as follows: pelvic bone resection using a Lumic type endoprosthesis was performed in 23 patients; proximal femur resection with endoprosthetics – 124; distal femur resection with endoprosthetics – 108; proximal tibia resection with endoprosthetics – 75; distal tibia resection with endoprosthetics – 6; long bone diaphyseal resection – 12 patients. Results and discussion. The study included 348 patients who underwent surgical treatment with endoprosthetics of the resected bone section. In the group using the perioperative rehabilitation program (the first group), which included 167 patients, the functional results were better than in the control group and amounted to 81.2%. Mechanical complications were detected in 18 patients (10.7%). The average functional results in 181 patients of the second group who did not receive a full perioperative rehabilitation complex were 71.8%, mechanical complications were in 13.25% (24 patients). Conclusion. Full perioperative rehabilitation is an important component, it significantly improves the functional result of the operated limb, promotes earlier activation, restoration of self-care in the shortest possible time after surgery. Improvement of functional results in patients also reduces the risk of mechanical complications.
