Аннотация

Введение. Рак молочной железы - одно из самых распространенных онкозаболеваний во всем мире, оно поражает женщин всех возрастов, занимает первое место по заболеваемости и третье по смертности среди злокачественных новообразований в общей структуре. Около половины всех больных раком молочной железы составляет пожилая и старческая группа. Цель исследования. Оценить пятилетние результаты у больных раком молочной железы в пожилом и старческом возрасте. Материалы и методы. В исследование было включено 180 больных раком молочной железы пожилого и старческого возраста. Пациенты были разделены на три группы по методу лечения: только хирургическое удаление, комбинированное лечение (операция + послеоперационная лучевая терапия), комплексное лечение (хирургическое лечение + послеоперационная химиотерапия + послеоперационная лучевая терапия). Результаты и обсуждение. Количество больных пожилого возраста (от 60 лет до 74 лет включительно) составило 133 (73,8 %), старческого возраста (от 75 лет до 90 лет) — 47 (26,2 %). По результатам исследова ния, установленным методом Каплана Мейера, безрецидивная и об щая выживаемость достоверно ниже у больных, к которым применили хирургическое лечение в монорежиме, что составило 88 % и 77,3 %, в сравнении с комбинированным лечением (хирургическое лечение + послеоперационная лучевая терапия) – 100 % и 82,5 %, а также в сравнении с комплексным лечением (хирургическое лечение + послеоперационная химиотерапия + послеоперационная лучевая терапия) - 90,8 % и 80 %, соответственно. Что касается объема оперативного вмешательства, общая выживаемость и безрецидивная выживаемость вы- ше при радикальном удалении молочной железы с регионарной клетчат кой. Заключение. Общая и безрецидивная пятилетняя выживаемость при комбинированном и комплексном лечении была достоверно выше в сравнении с только хирургическим удалением.
