Детерминанты показателей сывороточного 25(OH) D коренных жителей Кыргызстана

Аннотация

Статья впервые описывает детерминанты колебаний средних показателей уровня сывороточного 25(OH)D у коренных жителей Кыргызстана. Актуальность. Витамину D в настоящее время уделяется все большее внимание. Дефицит 25(OH)D способствует развитию как скелетных (рахит, остеопения, остеопороз), так и не скелетных патологических эффектов (дерматозов, гипертонии, ожирения, онкологических, эндокринных, аутоиммунных и инфекционных заболеваний). Уровень сывороточного 25-гидроксивитамина D> 75 нмоль / л, или 30 нг / мл, является необходимым условием для поддержания здоровья человека. Цель исследования- выяснить, являются ли пол и сезонные колебания уровня УФО детерминантами дефицита витамина 25(OH)D среди здоровых коренных жителей Кыргызстана. Материал и методы. Проведено пилотное многоцентровое кросс-секционное исследование уровня сывороточного 25 (OH) D коренных жителей Кыргызстана (41° с.ш., 75° в.д.; продолжительность солнечного сияния 1700- 2900 час/год). Выборочная совокупность составила 218 человек, из них 99 (45,4%) мужчины и 119 (54,6%) женщины. Средний возраст репрезентативной выборки составил 38 ± 1,0 (95% ДИ 36,0 – 40,1) лет. Результаты. Средний уровень сывороточного 25 (OH) D3 в общей выборке составил 46,9±1,1 нмоль/л (95%ДИ 44,8 - 49,1 нмоль/л), что соответствует дефициту (<50 нмоль/л). При этом у мужчин средний показатель 25 (OH) D3 составил 50,0 ± 1,5нмоль/л (95% ДИ 46,9 - 53,0 нмоль/л), у женщин 44,5±1,5 нмоль/л (95% ДИ 41,5 – 47,5 нмоль/л), р<0,013. Отмечены статистически значимые (р<0,026) сезонные колебания сывороточного 25 (OH) D3 от 41,7 ± 2,3 (95%ДИ 37,1 – 46,4) в весенний период до 50.0 ± 2.1 (95% ДИ 45,9 – 54,2) осенью. Вывод. Вне зависимости от статистически значимых гендерных и сезонных колебаний, а также географического расположения страны (41° с.ш., 75° в.д.) и продолжительности солнечного сияния (1700-2900 час/год.) средний показатель сывороточного 25 (OH) D3 не достигает адекватного уровня (>75 нмоль/л).
