Корутунду

Тери рагына чалдыгуу дүйнө жүзү боюнча тынымсыз өсүп жатат, тери рагына чалдыккандардын 90% дан ашыгы меланома менен байланышкан. Меланомада аман калуу шишиктин калыңдыгынан көз каранды. Ошондуктан, эрте аныктоо прогнозду жакшыртуу үчүн абдан маанилүү кадам болуп саналат. Акралдык меланома - алакандын, тамандардын жана тырмак төшөктөрүнүн терисинде пайда болгон меланоманын белгилүү бир түрү. Акралдык меланома - азиялыктарда меланоманын эң кеңири таралган түрү. Акралдык-лентигиноздук меланома менен ооруган бейтаптар, адатта, метастаздын жогорку ыктымалдуулугу менен оорунун өнүккөн стадиясында диагноз коюшат. Меланоманын башка түрүнө салыштырмалуу акралдык меланомада өлүмдүн көбөйгөнүн да- лилдер бар. Бул оорунун негизги өзгөчөлүктөрүн түшүнүүдөгү прогресске карабастан, акралдык меланоманы алгачкы этапта аныктоо кыйынчылык бойдон калууда. Азия тектүү адамдарда оорунун алгачкы стадияларында акралдык лентигиноздук меланоманы ийгиликтүү жана өз убагында диагностикалоо, ALM менен ооругандарды эрте аныктоо жана өз убагында дарылоо үчүн клиникалык, дерматоскопиялык, гистологиялык жана молекулярдык маалыматтарды интеграциялоо зарыл болуп калат, бул оң таасирин тийгизет. жашоо көрсөткүчү.

ОЦЕНКА ИММУНИТЕТА В УСЛОВИЯХ ПАНДЕМИИ (кыргызча)

В Кыргызстане продолжается бесплатное тестирование на антитела к COVID для исследования. Нужны участники из регионов!

В семи областях Кыргызстана и г.Бишкек и г.Ош проходит оценка популяционного иммунитета населения к вирусу SARS COV-2 в условиях пандемии COVID-19, которую проводит Научно-производственное объединение «Профилактическая медицина» Минздрава и соцразвития республики совместно с группой исследователей из Санкт-Петербургского НИИ эпидемиологии и микробиологии им.Пастера. 

Информация о состоянии популяционного иммунитета необходима для планирования санитарно-противоэпидемических мероприятий, в том числе вакцинации населения против COVID-19, как в отдельно взятом регионе, так и в стране в целом.

Общее число волонтёров должно составить 10752 человека (не менее 2688 чел. по 4 регионам с соблюдением принципа их равномерного распределения по районным центрам области, по возрасту и полу). Исключаются лица, находящиеся на стационарном и амбулаторном лечении с ковидным диагнозом.

В г.Бишкек уже набран материал, нужны участники из регионов!

К этапам проводимого исследования относятся информационная компания, анкетирование и получение информированного согласия; забор и транспортировка крови, получение и сохранение плазмы; проведение лабораторного анализа и обработка результатов.

Для определения IgG-антител к вирусу SARS-Cov-2 в исследовании используются ИФА тест-системы производства России для качественного и количественного определения антител.

Лето на самой макушке, а коронавирус не отстает. Почему не оправдались надежды на солнышко, отчего в разных странах разная смертность, нужно ли делать тесты и откуда вообще взялась эта напасть? Сегодня наш собеседник — один из ведущих в мире эпидемиологов, доктор медицинских наук из Атланты (США), профессор, президент компании DiaPrep System Inc Михаил Фаворов.

До середины прошлого века человечество ничего не знало о коронавирусах

 — Михаил Олегович, с высоты своего опыта работы в очагах различных инфекций можете назвать этот коронавирус уникальным?

— Благодарю, что пригласили на беседу. Для меня очень важно объяснять реалии и природу этого явления, потому что мир насыщен всевозможной информацией, и многое из того, о чем пишут, не соответствует действительности.

Прежде чем говорить об уникальности коронавируса, мы должны понять, что любой патоген — это сформировавшийся микроорганизм, который вступает во взаимодействие с людьми — макроорганизмами.

Нельзя говорить, что вирус такой или сякой. Если вирус не находится внутри человека — это неживая материя. Он не способен ни питаться, ни размножаться. И только когда попадает внутрь макроорганизма, то есть в нас, он становится живым.

Поэтому говорить об уникальности вируса как такового бессмысленно. Уникальность в его взаимодействии.

До середины прошлого века человечество о коронавирусах ничего, по сути, не знало. Они не вызывали тяжелых заболеваний, поэтому ими особенно не занимались.

Первый прорыв в понимании серьезности вопроса произошел относительно недавно. С 2002 по 2004 год в Китае вспыхнула эпидемия тяжелого острого респираторного синдрома — SARS. Теперь его называют SARS-1. Я тогда много работал, поэтому к вспышке SARS-CoV-2 был подготовлен.

Без вмешательства людей вирус не был таким патогенным

 — Откуда этот вирус взялся на нашу голову?

— Коронавирусы не появляются сами по себе, не возникают из ниоткуда. Каждый раз это определенные закономерности и, как правило, изменения во внешней среде.

Взять, например, этот коронавирус. То, что он сам по себе попал на базар в Ухане, откуда началась эпидемия, это просто невозможно. Вирус нашли у летучих мышей, которые обитают за тысячи километров от Уханя. И привезли в этот город вместе с мышами для изучения в местном институте.

То есть, пока люди не полезли в пещеры, не нашли этих мышей, не вырвали их из природной среды и не начали экспериментировать, вируса не было.

Мы также знаем, что вирус имеет такое же отношение к мышам, как и к панголинам. Сам по себе у мышей он не был патогенным для человека. А вот когда встретился с вирусом панголины и рекомбинировал — обменялся генной информацией, то стал страшно заразным для людей. Понимаете, какое значение имеют изменения во внешней среде?

 — Как вирус мог вырваться из института?

— Разговоров вокруг этой темы много. Заболел сотрудник и пошел на базар за покупками. Или подопытную мышь кто-то тайком вынес и продал.

В Китае такие дикие животные используются в медицине. По слухам, стоимость мышей этого вида для лечения равна по весу стоимости золота. Вполне могу представить, что кто-то из технических сотрудников института вместо того, чтобы утилизировать зараженную мышь, понес ее на базар. У людей остается коррумпированное мышление.

С точки зрения происхождения этот вирус действительно можно назвать уникальным. Теоретически он мог произойти без участия человека. Но вероятность того, что панголины обменялись генетической информацией с мышами, которые живут от них за тысячи километров, сами понимаете, насколько небольшая.

 Чем больше людей переболеют летом, тем легче будет осенью

 — В начале весны, когда мы входили в карантин, ожидали, что летом наступит затишье, что вирус будет гибнуть при высоких температурах. А он живет и живет.

— Вирус, несомненно, является сезонным. Но когда впервые попадает в популяцию, ему не до поры года — все восприимчивы, заражение идет легко. Есть еще один момент, и я о нем предупреждал, но не смог убедить.

С началом эпидемии были введены очень жесткие карантинные меры. Все сидят дома, заражения замедляются, все хорошо. Но долго держать население в изоляции не выходит. А как только отпустили — все бросились делать дела, которые из-за карантина пришлось отложить. Один поспешил зубы лечить, другой — колеса менять, третий пошел на работу в офис. Число контактов настолько возросло, что опять пошла вспышка, несмотря на то что лето и не сезон. Сейчас вирусу не очень комфортно, он не такой смертельный. И чем больше людей переболеют летом, тем выше будет коллективная иммунизация к началу ноября, когда начнется сезон.

 

 — Какая сейчас оптимальная модель поведения общества?

— Я считаю, что у вас оптимальная модель. Украина, безусловно, переживает стресс, нагрузку на систему здравоохранения. С другой стороны, возвращать сейчас жесткий карантин было бы неправильно.

Надо ориентироваться не по числу заболеваний. Число — это сколько человек обследовано, насколько хорошие и плохие использованы тесты. Надо ориентироваться на количество коек в больницах. Если их хватает, ситуацию надо держать.

 Профилактике научила «испанка»

 — Но вот уже в нашем Минздраве говорят, что разворачивают больницы второй очереди.

— Пока только разворачивается, это не плохо. Это не Испания, где не хватало крематориев. Надо все время следить за ситуацией в разных регионах. Регулировать не по стране, а по вспышкам.

Если сейчас все опять закрыть на жесткий карантин, вы ничего не выигрываете. Только ухудшите ситуацию в сезоне.

 — Сейчас все боятся второй волны. Это оправдано?

— Сезон коронавирусов немного сдвинут по отношению к гриппу: примерно с начала ноября до середины января. Вспышка 2020 года пришлась на конец сезона, но она наложилась на отсутствие любой иммунной прослойки. Говорить, что будет в сезон, сейчас трудно, не могу предсказать. Но не меньше, чем в начале вспышки — это да.

Я делаю сравнение с единственно возможной моделью — эпидемией «испанки», которая началась в 1918 году. В Украине тогда учета не было, кого застрелили, а кто умер от гриппа. Но в Америке был. Так вот после 1918 года, первого года пандемии, эпидемия 1919-го была в пять раз выше.

Однако с тех пор прошло сто лет. За это время и лечение стало лучше, и меры профилактики эффективнее. Ведь только в период «испанки» изобрели это социальное дистанцирование, впервые надели маски. Раньше ничего такого не знали.

 — На примере SARS-1 можно создать модель? Почему он не разошелся по миру? Вирус один, мутации — разные

— Такую модель делать очень сложно. Это разные вирусы, SARS-1 гораздо труднее передавался. Он был хуже приспособлен для передачи, чем SARS-CoV-2.

 — Есть версия, что разные народности воспринимают вирус по-разному. Одни — легче, другие — труднее.

— Нет, этого быть не может. Мы очень близки друг к другу. В нашем геноме нет ощутимого различия по расовым или национальным признакам. А те, которые есть, настолько минимальны, что вирус о них ничего не знает. Я эти басни слышу всю свою жизнь. Мы все одинаковые, вирус разный.

 — Так он не один, этот COVID-19?

— Вирус один, разные мутации, которые делают передачу более или менее легкой.

В европейской части мира циркулирует вирус с определенной мутацией, который более тяжелый. Такого нет — пока — в Юго-Восточной Азии. Поэтому в Южной Корее — 0,5% летальности, а в Италии до 20% в некоторых провинциях.

Но это временное отложение. В мире пойдет замена. На днях я посмотрел данные по Гонконгу. Там всполошились: ой, у нас опять пошел подъем, наверное, мы плохо зажимаем карантин. Да нет, это к вам приехал «новый» вирус — европейский.

Вирус не является однородным. Он изменяется, попадая в нашу среду. Он жил в мышах тысячу лет и не вызывал тяжелых болезней, а когда оказался среди людей, то действует совершенно по-другому, на другие рецепторы, другие клетки.

У вирусов есть участки, которые патогенно стабильны. А менее важные могут изменять их на подвиды от более тяжелого до менее тяжелого.

 Уже в два раза снизилась летальность

— Может такое случиться, что этот вирус мутирует в лучшую для людей сторону?

— Именно это и происходит. Вирус не наш, но он очень хочет с нами жить. А для этого должен прекратить убивать хозяина. Он все время старается стать легче и легче. Циркулируя среди людей, набирает все больше человеческих качеств, по сравнению с мышами.

И коронавирус уже в два раз снизил летальность. Такого кошмара, как был в Италии, больше нет. Потому что те вирусы, которые были очень тяжелые вначале, они всех своих хозяев поубивали и не передаются. А те, которые более легкие, будут передаваться.

Через пять-десять лет коронавирус вызовет у нас разве что легкий насморк, на который мы и внимания не будем обращать. Такая закономерность всех зоонозных инфекций.

 Лекарство есть, но только для тяжелых и только в Америке

 — А может быть лекарство от этого вида коронавируса?

— Во-первых, лекарство уже есть, его разработала фирма Gilead.  Но выпускают его пока только в США и только для США.

Это противовирусный препарат. Но проблема в том, что он не дает того результата, которого ожидали: быстрого и полного выздоровления. Летальность просто снижена с 11% до 8%, в два раза сокращается длительность болезни. То есть препарат действует, но не так хорошо, как хотелось бы.

— А это лекарство могут выпускать в планетных масштабах?

— Если у нас 7 миллиардов человек, нуждаться будет полмиллиарда, и я думаю, что таких производств пока не существует. Этот препарат и в Америке сейчас предназначен только для тяжелых больных, его нельзя применять при легких формах.

И США не желают его никуда продавать. Американские медики разделяют опасения перед сезонностью коронавируса.

 Не миф, но псевдознание

 — Еще к вопросу о мифах: писали, что восприимчивость к вирусу зависит от группы крови. Например, «первые» заболевает реже, чем «вторые».

— Любая болезнь некоторым образом связана с группой крови. Почему — мы не знаем. И чтобы это подтвердить, нужны большие исследования по группам населения. Вот вы знаете, какая у вас в Киеве группа крови главная? А во всей Украине?.. Даже если бы знали, то что-то смогли бы поменять?

Это не миф, но это псевдознание, которое ничем не поможет, зато может кого-то напугать. А психическое состояние в обществе и так подорвано. Молодые люди боятся выйти на улицу, чтобы не заразиться.

 — Когда, по вашим прогнозам, может появиться вакцина?

— Чтобы была вакцина, ее нужно испытать. И уже испытывают на добровольцах. Вот в России взяли батальон солдат и вводят им препарат. А дальше как? Будут эти люди защищены или нет, никто не скажет. Нужен опыт наблюдений.

Я раньше сезона 2021 года вакцину не ожидаю. По крайней мере для большого числа людей. Может быть, кому-то привьют из власть имущих или богатых, но появится ли она массово — вряд ли.

 — А может произойти так, что, когда вакцину окончательно одобрят, в ней уже и потребности не будет?

— И на этот вопрос сегодня никто не ответит. Мы не знаем, и я об этом уже говорил, какая будет ситуация в следующем году. Если сезон окажется большим, многие сами иммунизируются. Но потребность в вакцине есть, вакцина необходима, особенно для групп риска.

Широкое тестирование необходимо

 — Существуют ли люди, которые не заразятся, у которых природный иммунитет?

— Терминология не верная. Иммунитета нет. Есть люди с врожденной невосприимчивостью. Но какие тут действуют механизмы, наука тоже еще не знает, и таких людей нельзя выделить из популяции.

Зато мы знаем, что есть люди, которые переносят коронавирус без клинических форм, их в два-три раза больше, чем тех, кто имеет клинические формы — от легких до тяжелых симптомов.

Это касается не только COVID-19, есть люди, врожденно устойчивые к различным инфекциям.

 — Сегодня многие хотят сдать тест в надежде, что обнаружатся антитела. Есть в этом смысл?

— Конечно! Широкое тестирование необходимо. Тесты должны быть везде и доступны не просто для населения, но отдельно для организованных групп — армии, полиции, врачей в первую очередь. Хоть каждый день делайте тесты!

Почему? Потому что вчера тест был негативный, а сегодня может быть позитивный. Значит, садись дома и не распространяй инфекцию.

К сожалению, упор сейчас пошел на ПЦР-тесты, которые дают только половину подтверждений. Это неправильно. Надо прекратить делать массово ПЦР. Если будете здоровых тестировать, все выявленные случаи окажется ложно положительными.

Моя наука очень сложная, я не могу все объяснить в одном интервью. Но заключение простое: надо проходить тест на антитела. И если он положительный, тогда делать ПЦР.

 — Вы имеете в виду ИФА-тесты?

— ИФА или рапид-тесты (быстрые) хорошего качества на наличие антител. Потому что у бессимптомных больных эти антитела могут появиться за неделю до того, как ПЦР тест их покажет.

 — А как вы оцените наши украинские тесты?

— Украинские ИФА-тесты, которые мне удалось в нашей компании провести через международный стандарт качества, относятся к одним из лучших тестов в мире.

Сейчас хотим уговорить украинских специалистов сделать рапид-тест.  Я привез такие образцы турецкого производства. Могу испытать прямо на вас, результат в течение 15 минут.

 Маски — не самое тяжелое испытание

 — Спасибо, очень интересно. Но сначала последний вопрос: добрая часть украинцев считают, что коронавирус — это заговор масонов с «рептилоидами», а на самом деле ничего страшного не происходит. Что вы таким скажете?

— Я скажу: носите маски. Во-первых, маски уменьшают дозу вируса, которую можно получить от носителя, болезнь будет менее тяжелой. Во-вторых, и это главное, маски нужны, чтобы защитить других. Если вы не будете носить маски, вы заразите ваших стариков – отца, маму, тестя, тещу, и они умрут.

Да, не нравится, да, душно. Но маски — это не самое тяжелое испытание. Я четыре года работал в Корее. Там все надевают маски, когда заходят в метро. Они себя защищают, не хотят дышать теми паразитами, которые есть в толпе. А у нас это почему-то считается насилием над личностью.

И вот что еще скажу: разговоры про заговор хороши, пока ты не видишь самой болезни. А вот как только сосед заразится, как у него начнется пневмония, из которой месяц не сможет вылезти, вот тогда отпадает охота говорить о «рептилоидах».

То, что сейчас происходит в Украине, — это относительно низкая заболеваемость и низкая смертность. Цените! Проветривайте помещения, мойте руки, защищайте органы дыхания.

Никаких «рептилоидов» нет, вакцина — будет. Но сначала нам надо пережить тот сезон, который надвигается.

 Досье «КП» в Украине»

Уроженец СССР, доктор Михаил Фаворов сейчас живет и работает в Атланте (штат Джорджия, США). С 1993 года является членом Американского общества микробиологии, автор более 100 научных работ.

Известен как международный эксперт в области общественного здравоохранения и эпидемиологии. Работал в самых разных регионах мира на изучении и подавлении очагов редких видов инфекций.

Будучи региональным директором программы CDC в Центральной Азии, содействовал внедрению дозорного эпиднадзора за ВИЧ-инфекцией, гепатитом С и сифилисом в группах риска, внедрил программу универсальной вакцинации новорожденных от вирусного гепатита B, инициировал внедрение программы обеспечения и контроля качества лабораторной диагностики ВИЧ и туберкулеза.

На посту заместителя генерального директора Международного института вакцин (IVI) занимался внедрением программ вакцинации от брюшного тифа, вируса папилломы, холеры, гриппа, пневмококковой инфекции, менингококковой инфекции.

Научно-производственное объединение «Профилактическая медицина» будет заниматься определением антител к коронавирусу. Глава организации Омор Касымов ответил на вопросы Kaktus.media.

— Минздрав неоднократно заявлял, что намерен, как в Москве, начать проводить скрининговые исследования, чтобы выяснить, у кого в Кыргызстане уже выработан иммунитет к коронавирусу. Что это за исследования?

— С ВОЗ имеется соглашение для проведения скрининг-исследований с помощью серологических экспресс-тестов. Планируется, что ВОЗ предоставит до 5 000 тестов, сертифицированных в европейской зоне. В Москве проводили такой скрининг (ИФА-диагностика) и выяснили, что уже почти 20% жителей имеют антитела к COVID-19.

— Такого количества тестов хватит?

— 5 000 тестов достаточно. Мы возьмем разные группы из разных районов: разные возрастные группы, разные области. Есть специальная методика для экстраполяции и вычисления математическим методом процент населения, имеющий антитела.

— Почему мы сами не можем купить тесты?

— Законодательство таково, что тесты должны быть сертифицированы в нашей стране, а производители качественных тестов к нам их не завозят. У нас было три вида китайских тестов, мы дали отрицательный результат — это хлам настоящий. Ничего не дает.

— Когда начнется скрининг?

— У нас все для проведения исследования готово. Мы ждем поставку от ВОЗ.
